Ordering Information for Business Forms

How Long Will It Take To Get Our Forms Order?

Shipping or turn around time, on stock forms (if placed before noon) are generally shipped the same day. For custom forms it takes between 4 to 6 weeks. If a proof is needed (first time orders or a change has to be made) it usually takes 6 weeks provided proof is returned promptly.

May A Forms Order Be Cancelled?

Custom orders on forms may be canceled only if it has not gone to printing department. If a proof has be made there is a $25.00 cancellation charge and vendor keeps proof.

If member orders the wrong form and wants to return for correct form, member will be charged freight both directions and a restocking fee.

What Do I Do If There Is A Problem With My Order?

If you are not satisfied with forms please let us know what the problem is. Sometimes an adjustment will be made by the printer.

Merchandise returned must have an authorization or pick-up number provided by vendor. If package appears damaged, refuse delivery and contact MAPSA office.

Why Was I Charged Sales Tax On My Order?

The state of Ohio requires us to charge sales tax on all orders unless you have a properly filled out tax exemption form filed in advance with MAPSA.
